Caring Conversations® Materials

Caring Conversations Materials.

The Caring Conversations® materials will guide individuals and families through the advance care planning process with a highly individualized focus. The materials include forms for documenting who can speak for you and what treatment preferences you may have.

Advance Care Planning is a two-part process:

  1. Making your values and healthcare wishes known so you receive the kind of care and treatment you want.
  2. Ensure that those who speak for you are prepared to express your wishes. Goals of care discussions are vital to this process and emergency responders are sensitive to these goals, especially for patients living with advanced illness.

The workbook and introduction booklet are available in English or Spanish and includes a Durable Power of Attorney for Healthcare Decisions form and a Healthcare Directives form for your use.

Basically the workbook without the room to write your thoughts, but still contains the forms and guidance by answering the most frequently asked questions about this important process in 8 pages.

Caring Conversations Continued

Even if you have thought about your healthcare wishes, talked to your family and friends and completed the forms, circumstances change. There are specific decisions that may become critically important if you live with an advanced chronic illness or have been told you have a limited life expectancy. The TPOPP program booklet and form can help!
